Covid-19 Vaccine Announcement

Location: Dynasty Theater

Qualification: Everyone ages five and older
(People under 18 yrs need adult Guardian to be along with.)
Vaccines: Pfizer / Moderna
Accept all walk-ins. (First come, first serve, while supplies last).
For 1st dose

Driver License/Passport, Student ID/Birth Certificate and
-
Proof of Health Insurance (if any)
For 2nd dose
-
Must bring your CDC (Covid -19 Vaccination Record Card)
-
Pfizer 21 days after your 1st dose of vaccine.

Moderna 28 days after your 1st dose of vaccine.
For 3rd dose (Booster)
Everyone 12 years and older can get booster shots.
At least 5 months after completing your primary COVID-19 vaccination series

Must bring your CDC ( Covid -19 Vaccination Record Card)

Driver License, ID, or Passport and Health Insurance (if any) Required.

第二剂

必须携带您的 CDC（Covid -19 疫苗接种记录卡）

辉瑞公司在您接种第一剂疫苗后 21 天。

Moderna 在您接种第一剂疫苗后 28 天。
第三剂（加强剂）
每位 12 岁及以上的人都可以接种加强针。
完成主要 COVID-19 疫苗接种系列后至少 5 个月。

必须携带您的 CDC（Covid -19 疫苗接种记录卡）。

需要驾驶执照、身份证或护照和健康保险（如果有）。
